Organizer form

I heard you on John Williams talking about an organizer form. My understanding is that this form will have our personal/financial information for our family if we die. I couldn’t seem to find this form.
BTW, I heard you speak at a woman’s expo in Orland Park about 40 years ago and have loved listening to you since.
