With EBSA:
Online: Use the EBSA complaint form at ebsa.dol.gov or call 1‑866‑444‑3272 (Mon–Fri, 8:00 a.m.–4:30 p.m. local time)
By mail: Send to the EBSA office assigned to your plan’s location.
Confidentiality: Your name and complaint details will not be disclosed to your employer unless required by law

Go to labor.illinois.gov/file-a-complaint and choose the form for “Retirement or Pension Plan Issues”
Submit online, by email, or by mail with supporting documents.
