So that’s a form you receive from the GOVERNMENT — telling you how much they sent you (likely in unemployment benefits).
You should have received it by now — If you’re in Illinois, the only thing I can say is to call their toll free number at IDES to get put on the list for a callback.
Otherwise, if you can’t get through to them and know how much you were paid in 2020, include that as income on your tax return so you dont’ get a penalty.
