IDES issue Theft?

My wife’s employer received Notice of Claim to Benefit Chargeable Employer to deny or accept a request for unemployment for her from IDES that she never requested. He returned this stating that she was not unemployed, and did not file for unemployment, Our question is should we go thru all the steps listed on your website since this did not come to us, and we have received nothing ourselves. If we check her credit reports , should we do all three at once.
