After I pass what do my survivors do with the Transfer of Dead that has been registered in cook county court?

TOD designations are designed to avoid probate — passing the property (just the named property) directly to heirs, who must then go to the county registrar to transfer the deed into their name.
They must have proper documentation, so make sure you keep a record of everything.

Make sure you have correctly filled out the forms for the county in which the property is titled.
For example (and since many of my listeners/readers are in Cook County, Illinois), here is a copy of that specific TOD form. If the property is elsewhere, make sure you have the correct forms filled out.
