Hello Terry! Just saw you on the WGN morning show discussing wills and estate planning.
I completed a Five Wishes document (provided by Hope HealthCare Services) appointing an agent who can make medical treatment and healthcare decisions should I become incapacitated. It was witnessed by my sister and niece. Is a legal document in Illinois?

Yes, any witnessed document for healthcare directives will be legal in Illinois. You should have two copies — one for yourself that is easily available, and one in the hands of your agent.
