You're probably thinking of my Personal Financial Organizer form. You can get it by signing up in the yellow box that appears when you go to my website -- www.terrysavage.com. After you sign up, you'll receive an email asking you to confirm your subscription. Then after you do that you'll get another "thank you" email with a link to the organizer, which you can print out of fill out online and save to your own computer.
