When you sign up for my free newsletter -- which I write when the spirit moves me -- by filling out the yellow box on my website, you will get a return email asking you to confirm your subscription.  When you click on that email, you'll get a second email with the link to the form.  You can click on the link and fill out the form online and save it to your own computer, or print it out.  And you can forward the link to friends, so they can use it as well.
