Unemployment identity thief-IDES

On 2/2/21, I received a letter about UI benefits (zero monetary determination) at a place I never worked (Perry Memorial Hospital.) I immediately froze my accounts and checked recent (December-January) inquiries to make sure no cards were open in my name. Two phone numbers out of nine were dubious, one discontinued and the other one housed many credit cards. The others companies did not issue credit cards in my name.
The next day 2/3/21, I received an IDES “KeyBank” debit card. I tried to called the bank to state I did not ask for the card but the voice mail wanted the card number to activate it which I did not respond to. My questions what do I do with the card?
I also filed an IDES fraud report and will be filing FTC fraud report per FBI and your website.
I am 70 years old, retired for 4 years and worked for the USPS for 36 years. Credit score is over 800.

OK, just to make sure that you read and did EVERYTING on this list.

If your credit is frozen, and you are constantly watching your open accounts, then you are pretty safe. Now be on the lookout for a 1099 form stating you received benefits. The state is SUPPOSED to not send them if you filed a fraud complaint. If you can’t get it resolved with a corrected 1099, just don’t include it on your taxes. Keep track of your report to IDES. And put that card in a file with your other stuff in case you have to prove it to the IRS!
